Northwest Health is a regional healthcare network composed of three hospitals with more than 900 medical staff members, three ambulatory surgery centers, five emergency departments, and a multispecialty medical group with more than 140 providers who care for nearly 900,000 patients a year.

Northwest Health Family Medicine Residency Program’s inpatient experiences occur at Northwest Health – Porter and Northwest Health – La Porte, with the majority of inpatient rotation experiences occurring at our flagship hospital, Northwest Health – Porter.

Northwest Health – La Porte

Nestled in the county seat just off the shores of Lake Michigan in Northern Indiana, Northwest Health – La Porte has been the heart of healthcare in La Porte for more than 40 years. The 117-bed, four-story hospital provides acute care with strong programs in cardiology, orthopedics, surgical services, emergency care, oncology and maternal health. Just last year, Northwest Health – La Porte hospital delivered care to over 78,000 emergency department patients, 17,000 admitted patients, and our highly skilled surgical team completed 25,000 surgeries. Northwest Health Family Medicine residents will rotate to this site for clinical and educational experiences in general surgery, surgical subspecialties, and may have some FMP patients who choose the beautiful maternity center to deliver the newest member of their family.

Located in Knox, Indiana, Northwest Health – Starke is a rural, community hospital committed to providing quality healthcare and patient experiences to residents of Starke County and surrounding communities. Northwest Health – Starke offers adult acute care, emergency services, surgical services, orthopedics, gastrointestinal services, diagnostic imaging and cardiovascular testing. Outpatient programs include wellness and rehabilitation.

HealthLinc – Valparaiso

HealthLinc is the location for the Family Medicine Residency Continuity Clinic or “FMP”. HealthLinc is a not-for-profit Federally-qualified Health Center (FQHC) that provides medical, dental, optometry, and behavioral health services to people of all ages, regardless of their ability to pay. In 1996, HealthLinc began as Hilltop Neighborhood House, a neighborhood clinic and childcare center in Valparaiso, IN. Since then, HealthLinc has expanded across northern Indiana to 12 clinics, two telehealth clinics, and one mobile medical/dental clinic.

HealthLinc is committed to leadership, transformation, and innovation in health care. Residents are provided enhanced training in a community health center setting that grows their confidence, fosters their mastery of skills and allows them to gain competence in primary care practice. They provide intensive training on both the clinical complexity of family practice in the safety net setting and on a high-performance, Patient-Centered Medical Home model of primary care.

Northwest Medical Group Primary Care at Portage & Valparaiso Clinics

The Internal Medicine residency program residents are split as evenly as possible between two Northwest Health Medical Group locations, both of which are located within 20 minutes from Northwest Health – Porter. Once residents are assigned to either the Portage Clinic or the Valparaiso Clinic as PGY1s, they will continue practicing outpatient medicine at their assigned location for all three years of training. Located no more than 20-minutes from the primary training site, Northwest Health – Porter, supervising faculty will quickly become mentors to residents they supervise during the two-week continuity clinic experience (that occurs every four weeks). Note that both continuity clinics are closed Wednesday mornings so that residents can attend the Academic Half-Day in-person at Northwest Health – Porter.
